I give my permission for the student named above to participate in the Urban School-sponsored transportation services between the above listed pickup and drop off locations and Urban School via CYO-operated school buses. I agree to promptly sign the CYO School Bus Transportation Agreement and understand that my student is unable to ride the bus without the form being completed.

By signing this form, I am enacting my online electronic signature (for details, click here) and by submitting the document, am agreeing to pay all fees associated with purchasing bus services to and/or from the Peninsula/South Bay. I understand that the payments will be administered through FACTS Management Company.
