*New* Urban Letterman Jackets are here! A student may order his/her Letterman Jacket once he/she has completed at least one season of sport at the varsity level. The order form can be downloaded below or found in the Athletic Director's office.

Parent Credit Form - This is a one time credit where a parent can supervise a student's activity for one term. A minimum amount of hours is required. Contact the Athletic Department for more information.

Summer Credit Form - This is a one time credit where a student can receive credit for an activity over the summer. A minimum amount of hours is required. The summer before freshman year is not eligible. Contact the Athletic Department for more information.
